Before a clinician can serve North Carolina Medicaid members, whether through NC Medicaid Direct or one of the state’s managed care health plans, they must be enrolled with NC Medicaid through NCTracks. North Carolina charges its own state fee, so even individual practitioners pay to apply. Here is how North Carolina Medicaid provider enrollment works, step by step, as of 2026.

Who Runs Medicaid Enrollment in North Carolina

NC Medicaid is administered by the NC Department of Health and Human Services (NC DHHS). Providers apply, upload documents, and pay fees online through the NCTracks provider portal, and enrollment includes credentialing and license verification.

How to Enroll, Step by Step

The NCTracks individual enrollment user guide (September 2024) describes these steps:

  1. Get an NCID, the state login for the NCTracks secure portal.
  2. Check the Provider Permission Matrix for your provider type’s qualifications and required documents.
  3. Start the application by entering the ZIP+4 of your primary practice location, which sets in-state, border, or out-of-state status, then choose Individual or Organization.
  4. Complete each section, including service locations, taxonomies, group affiliation, EFT, sanction questions, and the Trading Partner Agreement.
  5. Sign and submit, then save the PDF of your application. The Final Steps page is the only chance to download it.
  6. Pay the $100 NC application fee.
  7. Upload required attachments and respond to any requests for more information.

Before You Start

  • An NPI. NPPES sends NCTracks a weekly update file, and a new NPI can take four to six weeks to link.
  • Your full legal name, Social Security number, and date of birth entered exactly. Errors can get the application withdrawn, and a new application means paying the fees again.
  • The documents the Provider Permission Matrix lists for your type, plus explanations of any work history gap of six months or more.
  • Bank details for electronic funds transfer.
  • For group practices, an affiliation for each clinician, by NPI and service location.

Screening and Fingerprinting by Risk Level

Federal rules require NC Medicaid to screen every applicant as limited, moderate, or high risk.

  • Moderate and high risk: a federally mandated site visit, performed for NC Medicaid by Public Consulting Group (PCG).
  • High risk: fingerprint-based criminal background checks for the provider and every owner with 5% or more direct or indirect interest. Medicare fingerprinting confirmed in PECOS can be accepted instead.

Notices go to the Office Administrator’s email in NCTracks, and missing the deadline means denial or termination.

Is There an Application Fee?

Yes, for individual practitioners too. North Carolina law requires a $100 NC Medicaid Provider Application Fee for enrollment, re-enrollment, and re-credentialing applications, including ordering, prescribing, and referring enrollments. It is in addition to the federal fee. The federal fee is $750 for calendar year 2026 and applies to institutional providers. Individual physicians and non-physician practitioners do not pay it.

Managed Care: Standard Plans and Tailored Plans

All providers must be enrolled in NC Medicaid before a health plan will consider them for a contract. Contracting is a separate step with each plan. Standard Plans cover physical health, pharmacy, and basic behavioral health. They are AmeriHealth Caritas North Carolina, Healthy Blue, UnitedHealthcare Community Plan, and Carolina Complete Health, which merged with WellCare of North Carolina on April 1, 2026. Tailored Plans serve members with serious mental illness, severe substance use disorders, intellectual and developmental disabilities, or traumatic brain injury. They are Alliance, Partners, Trillium, and Vaya, and only one serves each county.

Behavioral Health Clinicians

Yes, North Carolina enrolls behavioral health clinicians individually. Under Clinical Coverage Policy 8C, LCMHCs, LCSWs, LMFTs, licensed psychologists, licensed psychological associates, LCASs, and the associate-level LCMHCA, LCSWA, LMFTA, and LCASA licensees must each be direct-enrolled with their own Medicaid Provider Number and NPI. They cannot bill “incident to” anyone. Associate-level clinicians need a written service order from a physician, doctoral-level psychologist, NP, or PA by the first date of treatment. Dentists

Dental services are carved out of NC Medicaid Managed Care and covered by NC Medicaid Direct, so members can see any dentist enrolled with NC Medicaid without a health plan contract.

Ordering, Prescribing, and Referring Providers

NCTracks offers an OPR Lite application for ordering, prescribing, and referring providers who do not intend to file claims. The $100 fee applies.

Re-credentialing

NC Medicaid requires re-credentialing, also called re-verification, every five years. The invitation arrives in your NCTracks inbox, and you have 70 days to finish. Reminders go out 50, 20, and 5 days before the due date. If the application and $100 fee are not in by 5 p.m. on the due date, your record is suspended and claims pend. After 50 days of suspension, you are terminated. Out-of-state providers must instead complete full enrollment every 365 days.

How Long It Takes

NC Medicaid reports average turnaround times for clean enrollment and re-enrollment applications of 15.9 days in July 2026, 16.94 days in August, and 22.06 days in September. An effective date can go back up to 365 days before a complete application is submitted.

Common Reasons Applications Stall

  • A legal name, Social Security number, or date of birth that does not match, which can get the application withdrawn.
  • Missing the 30-day deadline for requested documents, or the two 10-day follow-ups, which gets the application abandoned.
  • Applying before a new NPI has linked in NCTracks.
  • Fingerprinting not completed by the stated deadline.
