In North Carolina, every clinician pays to apply for Medicaid: a $100 state fee charged on enrollment, re-enrollment, and every five-year re-credentialing. It also splits managed care between Standard Plans and Tailored Plans, and lets therapists, including associate-level licensees, enroll individually. Maximum Billing provides credentialing services for North Carolina practices, covering NCTracks enrollment, health plan contracts, commercial plans, Medicare, and CAQH.

Enrolling Through NCTracks
The NC Department of Health and Human Services administers NC Medicaid, and providers apply, upload documents, and pay online through the NCTracks portal with an NCID login. Accuracy matters more than usual here: an error in a legal name, Social Security number, or date of birth can get an application withdrawn, and starting over means paying the fee again. Requested documents are due within 30 days, with two 10-day follow-ups before an application is abandoned. Our North Carolina Medicaid provider enrollment guide walks through NCTracks.
Standard Plans and Tailored Plans
NC Medicaid enrollment comes before any health plan contract, and each plan contracts separately.
- Standard Plans (physical health, pharmacy, basic behavioral health): AmeriHealth Caritas North Carolina; Healthy Blue; UnitedHealthcare Community Plan; Carolina Complete Health, now combined with WellCare of North Carolina as of April 1, 2026.
- Tailored Plans (members living with serious mental illness, severe substance use disorders, intellectual or developmental disabilities, or a traumatic brain injury): Alliance, Partners, Trillium, and Vaya, each covering its own counties. A Tailored Plan can close its mental health and substance use network to new providers.
Behavioral Health Credentialing in North Carolina
Clinical Coverage Policy 8C has each of these clinicians enroll directly: LCMHCs, LCSWs, LMFTs, LCASs, licensed psychologists and psychological associates, and the associate-level LCMHCA, LCSWA, LMFTA, and LCASA licensees. Each one enrolls with their own Medicaid Provider Number and NPI, and none can bill “incident to” another clinician. An associate-level clinician also needs a written service order, signed by a physician, a doctoral-level psychologist, an NP, or a PA, in place by the first treatment date. See our mental and behavioral health credentialing services and North Carolina mental health billing services.
Dentists
Because NC Medicaid Direct, not the managed care plans, covers dental care, a dentist enrolled with NC Medicaid can see members without a health plan contract. See our dental credentialing services.
Re-credentialing and Timelines
North Carolina re-credentials every five years. The invitation arrives in the NCTracks inbox with 70 days to finish, and reminders follow at 50, 20, and 5 days. Missing the 5 p.m. deadline suspends the record and pends claims, with termination after 50 days of suspension. On the positive side, NC Medicaid reported average turnaround of 15.9 to 22.06 days for clean applications in July through September 2026, and an effective date can reach back up to 365 days.
Frequently Asked Questions
Do individual clinicians pay a fee for NC Medicaid?
Yes. North Carolina charges a $100 application fee for enrollment, re-enrollment, and re-credentialing, including ordering and referring enrollments.
Can associate-level therapists enroll in NC Medicaid?
Yes. LCMHCAs, LCSWAs, LMFTAs, and LCASAs enroll directly, and they need a written service order by the first date of treatment.

Standard Plans handle physical health, pharmacy, and everyday behavioral health. Tailored Plans are for members with more serious behavioral health needs, intellectual or developmental disabilities, or a brain injury.
