South Carolina screens independent behavioral health clinicians as high risk, which means fingerprints, a site visit, and a required orientation before a therapist can even apply. It also limits enrollment to a service area that reaches only 25 miles past the state line. Maximum Billing provides credentialing services for South Carolina practices, covering Healthy Connections Medicaid enrollment, managed care plans, commercial plans, Medicare, and CAQH.

Healthy Connections Medicaid Enrollment
The South Carolina Department of Health and Human Services (SCDHHS) runs Healthy Connections Medicaid and handles enrollment itself, without a separate fiscal agent. Starting the online application issues a Reference ID, and an in-process application is purged after 30 days. Four documents are signed electronically: the participation agreement, the EFT agreement, the W-9, and the Trading Partner Agreement. Applicants should wait for official notice before treating members. Our South Carolina Medicaid provider enrollment guide explains each step.
The South Carolina Service Area
Unless SCDHHS approves otherwise, providers have to be located in the South Carolina Medicaid Service Area: South Carolina itself plus the parts of North Carolina and Georgia within 25 miles of the border. Border-area providers follow their own state’s licensure rules. Practices further away receive out-of-state status and can enroll only as state or federal law allows, which matters for telehealth practices based outside the state.
Managed Care Plans
Contracting with a Healthy Connections plan does not replace state enrollment; SCDHHS requires both, even for managed-care-only providers. The five plans SCDHHS lists:
- Absolute Total Care
- Healthy Blue by BlueChoice
- Humana Healthy Horizons
- Molina
- First Choice by Select Health
Behavioral Health Credentialing in South Carolina
Psychologists, LISW-CPs, LPCs, LMFTs, and licensed psycho-educational specialists each enroll on their own first, then link to a group, which carries a separate NPI and Medicaid ID. Because they are screened as high risk, they go through fingerprinting with SLED and the FBI and a pre-enrollment site visit, and before applying they must pass a pre-enrollment orientation, scoring 90% or higher on its knowledge review. Work by clinicians without an independent license is billed through an enrolled supervisor in the same discipline. See our mental and behavioral health credentialing services and South Carolina mental health billing services.
Dentists and Ordering Clinicians
DentaQuest administers dental benefits as SCDHHS’s dental vendor, but dentists still enroll with SCDHHS first. Clinicians who only order, refer, or prescribe can use the abbreviated Ordering/Referring enrollment, which cannot bill; starting July 1, 2026, fee-for-service claims for ordered or referred services began rejecting when the ordering or referring NPI is missing. See our dental credentialing services.
Revalidation and Timelines
SCDHHS revalidates every five years, or three for durable medical equipment providers, and terminates providers who don’t submit on time; its behavioral health manual also mentions yearly revalidation for solo practices, so we confirm each schedule with Provider Enrollment. Applications are processed within 30 business days of receipt, longer with a site visit or contract, and an approved enrollment takes effect 90 days before SCDHHS received it.
Frequently Asked Questions
Why are South Carolina therapists screened as high risk?
SCDHHS places independent behavioral health clinicians in the high-risk category, which adds fingerprinting and a pre-enrollment site visit.
Is there an orientation before a therapist can apply?
Yes. Therapists must pass a pre-enrollment orientation, scoring 90% or better on the knowledge review, before they apply.
Can an out-of-state practice enroll in South Carolina Medicaid?
Providers within 25 miles of the border in North Carolina or Georgia are in the service area. Others receive out-of-state status and can enroll only as law permits.
Which Healthy Connections plans do you credential with?
All five: Absolute Total Care, Healthy Blue by BlueChoice, Humana Healthy Horizons, Molina, and Select Health’s First Choice.
